Form responses

Read what a customer answered, find a single submission fast, and understand which answers also land on their profile.

When you need to check what a customer told you before they arrive — their pet's breed, an allergy, a consent box, a phone number they typed in — this is where every submitted answer lives. You open it per form, so you are always reading the answers to one form at a time, with every submission kept on record.

Open the answers for a form

In the left navigation open Clients → Forms, find the form, and click View Responses. The button shows the submission count in brackets, so you can tell at a glance which forms have answers waiting.

Scan everyone's answers side by side

The table builds one column for every question on the form, plus a first column for who submitted and a last Submitted column with the date and time. Reading down a single question's column lets you compare answers across customers — every "Breed", every "Allergies" — without opening anything.

Answers that were multiple choices show joined together (for example "Sensitive skin, Anxious"). A long answer is shortened to fit its cell, so anything that looks cut off has more behind it — click the row to read the full text.

Find one submission

The table shows 20 submissions per page, newest first, with page controls at the bottom. For a form with a handful of responses you can see everything at once; for a busy form, step through the pages to reach older submissions.

To read one in full, click its row rather than relying on the search box — clicking is the reliable way to land on a specific person's answers, and it opens their complete detail panel (below).

Read a single response in full

Click any row to slide open the Response Details panel on the right. It shows the exact submitted date and time at the top, then two sections:

  • Client Information — the submitter's name, plus their email and phone when they gave them. Email and phone rows only appear if that detail exists, so a sparse panel just means the customer left those blank.
  • Form Responses — every question on the form paired with the answer, with multi-select answers joined by commas.

Tell apart a logged-in customer from a guest

A submission from someone with an account shows their name as the client. A submission from someone without an account shows as a guest instead of a named client — the answers are still captured in full, you just won't see a linked profile for them.

This is the quiet detail behind "the form was filled but I can't find the customer": they completed it as a guest, so there's a response here but no client record to open.

Understand which answers also reach the profile

Some forms are Profile forms — they're set up to build a customer record (a pet, a vehicle, a person) from a designated name field. When a customer submits one of those, the structured answers — breed, weight, vaccination status — are copied onto the matching client profile as clean, editable values, while the raw wording stays here.

So there are two copies on purpose: the original submission on this page, and a tidy structured version on the profile. To correct a value going forward, edit it on the profile; this page is the untouched record of what was originally sent.

Tips

  • A form keeps its responses safe by refusing to be deleted. Once a form has even one submission, its Delete option on the Forms page is disabled — the answers (often consent records) can't be wiped out by removing the form. Stop a form from collecting new answers by deactivating it instead.
  • A Profile form creates a new row each time it's submitted — a customer adding a second pet to the same form leaves two separate submissions, both kept on record.
  • Multi-select and long answers aren't truncated in the detail panel — only in the table cells. If a table answer looks clipped, open the row for the whole thing.
  • Need only the clean values, not the raw submission? Open the customer from the client list and read their profile — you don't need this page for that.
